LVHS Alumni Poll Suggestion Form
Use this form to suggest future LVHS Alumni Polls. The Web Master reserves the right to refuse and/or edit all suggested polls, as well as decide the order in which accepted polls are posted. If a valid E-Mail address is given, an acknowledgement of receipt will be issued, and your poll suggestion will be considered. Polls will be posted anonymously; only the Web Master will know who submitted the poll.
The poll question should be as short and clear as possible. There is a minimum of two "multiple choice" type response options. The current maximum number of response options is fourteen, though it is suggested you not get to complicated and/or numerous with response options.
